Massachusetts Aging Service Access Points

FOCUS ON Frail Elders and Caregivers

  • Provide community based services through contract, to enable elders to remain in the community
  • Conduct assessments with elder and family
  • Determine eligibility for state-funded programs
  • Provide care management and client advocacy
  • Manage Protective Services, Crisis Intervention, and Elder at Risk Programs
  • Provide Information and Referral
  • Conduct medical screenings for Medicaid services, including nursing facility screenings
    * Certain State funded programs have eligibility guidelines
